We understand that filling out forms can be frustrating so we’ve made ours as easy as possible to complete. We’re sure you understand that as we work with children we have to be thorough in the information we hold which means the form is a bit long.  We are committed to making the enrolment process as inclusive as possible. If you need assistance in completing this please email us and we will do our upmost to help you.  We’d suggest setting aside 5 minutes or so to read and complete the form fully.   This form doesn’t ask for any payment information. We’ll provide you with a Direct Debit form once your child’s place has been confirmed.
